To use medical marijuana legally in Arizona you must have one of the qualifying conditions. And unfortunately this list is very small compared to other states. For example, in New Mexico, people with Post traumatic Stress Syndrome qualify for medical marijuana; in California, severe anxiety and insomnia qualify.
However, the Arizona Medical Marijuana Act that was passed in 2010 requires the State to reevaluate that list once a year by accepting petitions from the public about which conditions the voters think should qualify.
Thankfully, that time is almost upon us. From January 23rd to the 27th, the Arizona Department of Health Services will accept petitions for additional debilitating medical conditions to be added to the list of qualifying conditions.
So, if you think your condition should qualify you to use medical marijuana, download this form. Fill it out and mail it in to the AZDHS at the address listed on the form. Do not submit it BEFORE OR AFTER the dates indicated though, the AZDHS seems to be doing everything in its power to make this process as difficult as possible and isn’t accepting petitions postmarked outside those dates. (You can probably thank Governor Brewer and Will Humble for that)
