Contact the Arizona Dept of Health Services Medical Marijuana Program

If you have questions about the Arizona medical marijuana program or your Arizona medical marijuana card is lost or stolen, you’ll need to contact the Arizona Department of Health Services (ADHS).

If you know your medical marijuana card number you can visit the ADHS’s website and complete their provided form. If you do NOT know your medical marijuana card number, you have to email the ADHS’s medical marijuana support email address.
